Who is eligible for Ho`ole`a?
Anyone who 18 or older who signs up with a valid, active email address and full name and confirms is considered a member of Ho`ole`a.

Rewards for completing our questionnaire. 
Upon submission of the questionnaire form you may download your $25 loyalty reward gift certificate from the confirmation screen. The gift certificate may be used towards any non-medical, cosmetic product or service available at all three Cosmetic Centers of Hawaii locations.

You must have valid ID that matches the name entered in the questionnaire form to redeem the gift certificate so be sure to bring it with you. The gift certificate must be redeemed before the expiration date. You must be 18 or older. One gift certificate per person and must be used by the recipient.

Your email address will automatically be entered for frequent drawings of products and services throughout the year. Winners will only be notified by email so be sure to add support@drjohnstover.com to your address book. Gifts and prizes must be claimed within three weeks of the drawing date and are non-transferable.
